How is it possible to embed a flash application (.swf file) as the background of the form. So my question is ...

1.  What are the OLE that I need?
2.  How can I set the flash file as my form's background?
2.  Do I need to type anything in the code below?

You can't exactly put flash as the background, however you can insert the ActiveX control that allows flash and then press CTRL+K to send it to the back. The Flash control can be found by opening the components dialog box and selecting "shockwave flash". If you need any further explanation just reply to this post.

What do you mean by Ctrl-K to send it to the back? Sorry I don't understand. Can u pls elaborate more?

CTRL+K as in the keyboard shortcut. The other way of doing what i suggested would be to click on the flash control and press the menu item shown below on the screenshot:
