how can you make the dialog appear with the new folder button and the textbox for manually inputing the directory?

This type of dialog doesnt have new folder buton, nor it can be placed by changing some of the code. This code of yours is displaying network neighbourhood, and it can be changed to display local directories, but with no New folder button. If you want to have new folder button, you would have to make your own control, using DriveListBox and DirListBox and then you can control creating a new folder from your code easily.
